Immunizations and Medications

 

Immunizations

Washington immunization law requires that all students attending public schools, preschool through 12th grade, must provide proof of immunization, present a medical exemption signed by a physician, or a personal/religious exemption signed by a parent/guardian, before entering school.    It is the parent/guardian’s responsibility to provide this information to the school.  For additional information, contact Anna Hoxie at (360) 273-4946.

 

Vaccine

School Nurse

The Oakville School Nurse provides:

  • Skilled emergency care
  • Monitor administration of medicine and provide instruction to staff
  • Case management planning with parent/guardians to provide health services for short term and/or complex health issues
  • Counseling for a healthy life-style
  • Advocacy for the child in school
  • School safety
  • Information on current health issues
  • Instruction on health issues

Hepatitis B

3 doses for preschool, kindergarten, 1st and 2nd grades

3 doses beginning September

DTP – Diphtheria Tetanus Pertussis

4 doses, provided that last dose was on or after the child’s 4th birthday

4 doses of DTP, DT or TD, provided that the last doses was on or after the child’s 4th birthday

Polio

3 doses, provided the last dose was on or after the child’s 4th birthday

3 doses, provided the last dose was on or after the child’s 4th birthday

Measles

1 dose live vaccine on or after the child’s 1st birthday

2 doses live vaccine on or after the child’s 1st birthday and at least

30 days apart.

Mumps

1 dose on or after the child’s 1st birthday

2 doses on or after the child’s 1st birthday and at least 30 days apart.

Rubella

1 dose on or after the child’s 1st birthday

2 doses on or after the child’s 1st birthday and at least 30 days apart.

Chicken Pox

 1 dose or proof of immunity (kindergarten only.)

1 dose or proof of immunity

(sixth grade only.)

 

Student Illness or Injury

If a student becomes ill or injured, it is important that the school office know how to contact parents/guardians during the school day.  Parents/guardians are asked to keep the school office current on both, home and work telephone numbers  When numbers change please notify Anna Hoxie immediately.

 

Health Screening

Students in kindergarten through 5th grade are weighed and measured each year.  In addition, kindergarten, 1st, 2nd, 3rd, 5th, and 7th grades are tested for vision and hearing.  Students in 5th and 7th grade are also screened for scoliosis (curvature of the spine).  Parents are notified of possible problems indicated by the screening program and recommendations are made for further evaluation by a physician.  Families for whom further medical evaluation poses a financial hardship contact the school nurse or Anna Hoxie as (360) 273-5946 to discuss such assistance.

 

Medications

State Law and District policy govern the administration of medication to students.  Students are not permitted to attend school without proper forms.  For those students who require medication at school, the school office can only accept written and signed authorization from the parent/guardian and the prescribing physician.  This applies to all over-the-counter medications as well as prescription medications.  Medications must be brought to the school in its original container and labeled with the student’s name, dosage and frequency instructions.  Medications will be kept in the school office at all times.
